Faculty/Staff Tuition Waiver Notification Form

This form may only be used to notify the Cashier's Office of your intent to use the Faculty/Staff Tuition Waiver. The UCO will credit your account for the Tuition and Fees upon receipt of your completed and signed paper tuition waiver form and approval by Human Resources.


Employees MUST enroll in the course by the Census Date in order to use the tuition waiver:

Summer Session I - May 19, 2017
Summer Session II - June 28, 2017

STOP! THIS DOES NOT COMPLETE THE PROCESS! APPROVED PAPER FORMS (NOT INCLUDING THIS NOTIFICATION FORM) RECEIVED AFTER THE DEADLINE WILL BE DENIED.
